In accordance with tax law and as defined by the Internal Revenue Service, if you received or desire to receive any of the following, you agree that you are eligible for, as summarized in the following pages of questionnaires and checklists:

• Economic Impact Payment (Recovery Rebates)
• Earned Income Tax Credit (EIC)
• Education Tax Credits
• Child Tax Credit (CTC) or Additional Child Tax Credit (ACTC)
• Credit of Other Dependents (ODC)
• Head of Household (HOH)
• Self Employed (non-hobby business)
• Other tax credits for future tax years not listed above

BY SIGNING, SUBMITTING, AND/OR OTHERWISE ACCEPTING ANY FORMAL AGREEMENT WITH US (INCLUDING PROVIDING YOUR PAYMENT INFORMATION) YOU HAVE ACCEPTED THE TERMS OF THIS AGREEMENT AS WELL AS ACKNOWLEDGE THAT YOU HAVE READ AND AGREE TO OUR “TERMS OF SERVICE AGREEMENT”, “TERMS AND CONDITIONS”, “PRIVACY POLICY” AND “ACCEPTABLE USE POLICY“.
